Valley Intermediate School Registration Guide

The following instructions will guide you through the registration process for your child. Remember to print your completed forms and bring them to registration. We recommend using Internet Explorer to view and print these forms. Other browsers may not allow you to print the forms once you complete them. If you use a browser other than IE, please use the most current version. These forms have tested successfully  on IE 7, IE 8, Firefox 10 and Safari 4.1.3.

2012-2013 Registration Information

Students NEW to Shelby County Schools

  • Print your new student registration check list first. Click here to access check list
  • You may complete general registration documents. Go to Forms.
  • Serious Medical Condition or Food Allergy Form - All Parents must complete this form at registration
    Medical-Food Allergy Form Click here for SMC-FA form
  • Bring on-line registration forms and the following documentation (birth certificate, 2 current proofs of residence and an Alabama Blue Immunization form) to the school registration on the date listed above. For other documents needed, please refer to your Registration Packet Checklist.
    • New Student Registration - July 12, 2012 – 9:00 a.m. - 12:00 noon

My child is already a student at VAES and will be at VAIS in 2012-2013

  • Print your returning student registration check list first. Click here to access check list
  • Complete VAIS on-line forms here. Forms
  • Serious Medical Condition or Food Allergy Form - All Parents must complete this form at registration
    Medical-Food Allergy Form Click here for SMC-FA form
  • Complete and return your on-line registration forms or your courtesy registration packet to your school (VAES) by April 13, 2012. After this date, all other registration packets must be taken to Valley Intermediate School by April 19, 2012.
  • Bring (2) current proofs of residence dated July 2012 to Valley Intermediate School on your “summer turn-in” day during your child’s designated grade-level and time.
    • 3rd grade- July 19, 2012 – 8:00 a.m. - 11:00 a.m.
    All Parents will receive a “Fast Pass” on this day which will allow you to participate in our “Meet the Teacher Day”.
