On-Line Forms for School Registration

Complete and print forms required for enrollment.  We recommend using Internet Explorer to view and print these forms. Other browsers may not allow you to print the forms once you complete them. If you use a browser other than IE, please use the most current version. These forms have tested successfully  on IE 7, IE 8, Firefox 10 and Safari 4.1.3.

Do I have to use on-line forms for registration?

No. You may choose to complete registration paperwork provided by your school.

What do I need to use the on-line forms?
  • Your student's id information
  • Medication information for your child (if applicable)
  • Contact names and phone numbers for emergency contacts and those authorized to check the student out of school
  • Computer
  • Printer
  • Set your printer margins to .5 inches before you attempt to print the completed forms.
What school will my child attend?

The Shelby County School System is divided into zones which determine the school a child should attend. To learn which school your child will attend please visit the Zones webpage.


Is my child's information secure?

On-line forms for Registration do not transfer data across the Internet. If you receive the message "Uploading Data" at the end of the process, it means that it is uploading to your printer.
