Planned Absence Form

 Needed when you know in advance that your child will miss school. (regardless of the number of days)

Medication Form

 Please remember the office cannot administer any prescription medicine without a physician’s authorization.  Over-the-counter medication may be administered for only two weeks with a parent signature.  In order for Over-the-counter drugs to be taken all year, it must be authorized by the doctor.  Students are not allowed to transport any medicine to school.  A parent must bring in all medications.
